Insanity, Before or After?

I've had this image in my head for a while also. ...I've been feeling so oppressed lately, due to my circumstances, which I shall not divulge here because this is my world and it shan't be tainted by the mentioning of such things. Anyway, I wanted to express the idea that I am being held "hostage" by my duties (the fancy birdcage), and I am trying to make the best of my circumstances. I am going insane, so I can remain sane. But what if I was already insane?...
